Independent Community Care Management (ICCM) specialises in supporting people with complex conditions - at home, throughout the United Kingdom.

Our aim is to enable people to regain control of their lives following a major healthcare event. This is achieved by focusing on specific conditions and providing professional, responsive support networks.

ICCM is not an agency – this does not fit with our ethos and approach. All staff are employed, in order for ICCM to deliver a quality assured, person-centred service. This service is characterised by -

  • Each ICCM client, with their family, selects their own care team
  • Each client is always at the centre of decision making
  • An ICCM Case Manager helps clients develop, implement and regularly review their own care plan
  • Professional support is tailored to the client’s condition and needs
  • Investment in staff training and skills, which are responsive to each client’s needs

Many of ICCM's clients have complex conditions. They require ICCM carers to be skilled in advanced care or nursing techniques. As a consequence, ICCM is dual registered with the Commission for Social Care Inspection (CSCI), as a Nursing Agency and a Domiciliary Agency.
